Welcome to our Online Reading Form Submission! Simply fill out the form below, and your prize will be waiting for you the next time you visit the Children's Department! Remember, you earn prizes for the first three forms submitted, and your name is entered into our Grand Prize Drawings for each form you enter. Be sure to click the button at the bottom when you are finished.

The final day to submit reading forms is Thursday, July 29.

Lap Readers and Grades K-3: Please list the titles of 10 books read by the child on each form.

In-Be-Tweeners should submit one list of 10 books. If possible, list the names and dates of four activities attended as well. (Summer Volunteers need not list activities.)

If this is your first, second, or third form submitted, or if you are an In-Be-Tweener or Lap Reader, you may pick up your prize at the Children's Department.
If you have already submitted three or more forms (or if you are an In-Be-Tweener and have already submitted one form), you will continue to have your name added to grand prize drawings for each additional form you submit. So keep on reading!
